Equal Chance Limited Policy on GDPR – Privacy Information
This privacy information as part of the consent form sets out how Equal Chance Ltd. uses and protects any personal information that you provide us. Equal Chance Ltd. is committed to ensuring that individuals’ privacy is protected. Should we ask you to provide sensitive information or information by which a person can be identified, when using our services you can be assured that it will only be used in accordance with this privacy statement.
Why we collect your data
We collect clients and their family/carers personal data because you give us consent to do so and/or because we have a contract in place with you or a third party, in order to provide our specialist paediatric occupational therapy services to the client. We also have a legal obligation to keep clinical records.
What we collect
We may collect the following information:
Personal information about the client and their legal guardian and family members (name, date of birth, address, email, telephone)
Clients’ medical and lifestyle information, through conversation, observation and questionnaires;
Further clinical information in relation to reports from other health professionals.
What we do with the information we gather
We require this information for the purpose of:
Communicating with the client / their representative regarding their treatment;
Professional clinical record keeping of patient information;
Creating fully informed reports and treatment plans;
Sharing information with relevant health professionals

We are committed to ensuring that personal information is secure. In order to prevent unauthorised access or disclosure we have put in place highly secure electronic systems and managerial procedures to safeguard and secure the information we collect.
Controlling your personal information
We will not distribute, sell or lease personal information to third parties unless we have explicit permission or are required by law to do so.
Individuals may request details of personal information which we hold about them under the General Data Protection Regulation (GDPR). If you believe that any personal information, we are holding is incorrect or incomplete please email or write to us as soon as possible at the above address. We will promptly correct any information found to be incorrect.
Data Retention
We will retain client’s personal data for 7 years after they stop using our services, or 7 years after the age 18 for a child, unless there is justification for us to retain it for a longer period.
How you can withdraw consent
If you do not wish us to make use of your/your child’s personal information in this way please contact Abbie Hancock, or email abbie@equalchanceot.co.uk. You have the right to withdraw consent.
Right to Erase
You can of course request that your child’s records be erased in accordance with your right to erase data under data protection law. However your data is health and care data that has been processed under the responsibility of a health professional that has a legal obligation of professional secrecy which falls info one of those special categories where the right to erase does not apply. Furthermore, we do need to retain your information for insurance purposes. Please see data retention above.
